Where United States baby / infant passport photos are actually used
Shooting from above solves most of the problems at once. The thing to watch is your own shadow: standing between the window and the baby throws a shadow across the sheet, which reads as a coloured background. Use light from the side, or shoot near a window with the baby facing it. Note that a DS-11 for a child under 16 has to be filed in person, with both parents present or the correct consent form — the photo is only one part of it.
